We’re accepting new patients within our capacity. Your Ontario health card and a few documents help us build an accurate chart, coordinate referrals, and respect your time on day one.

BEFORE YOU ARRIVE

What to bring

Having these items reduces delays and helps your physician focus on your concerns.

Ontario health card

Valid OHIP coverage card (or documentation of status if you’re registering or renewing—we’ll guide you).

Government photo ID

To confirm identity when required for registration and certain forms.

Medications & allergies

Bring all prescription bottles or an up-to-date list—including over-the-counter products and supplements.

Records & referrals

Specialist letters, lab or imaging results, hospital discharge summaries, and any pending referral information.

Insurance & benefits cards

Extended health, drug plans, or workplace benefits—useful for non-OHIP services or dispensing coverage.

Book your appointment

Request a time online or by phone. Tell us if you’re new to the clinic and any urgent concerns.

Complete intake

Arrive a few minutes early for forms and ID verification. Bring your health card and medication list.

Meet your physician

Review history, goals, and preventive care. Ask questions—we explain options and OHIP coverage in plain language.

Coordinated next steps

Referrals, tests, prescriptions, or follow-up visits are planned and documented in your chart.

Registering with our clinic

We enroll new patients when our panel has capacity. If you’re new to Ontario or need help with OHIP status, we can outline the steps and what to bring to ServiceOntario.

Transferring from another family practice? Bring your previous clinic’s name, phone, and fax if available so we can request records when needed—this helps avoid duplicate tests and keeps your history complete.
